Tuckahoe Veterinary Hospital offers our patient form(s) online so you can complete it in the convenience of your own home or office.

  • Fax us your printed and completed form(s) or bring it with you to your appointment.

New Patient Registration Form - Required​​​​​​​

This let's us record pets and owners into our management software so that we can effectively and efficiently provide the service you deserve! Please complete the Client Information Sheet and fax it to (804) 741-7424 prior to you appointment. If you do not have access to a fax machine, you may bring the form with you to your appointment, but also please call the office ahead of time (804) 740-7600 with your information so that we can enter it into our management software and provide you and your pet with excellent service the moment you arrive.

Boarding Contract

We offer boarding for both dogs and cats. Please call 740-7600 for reservations at least one week in advance of you scheduled trip. Please be advised that we have limited space for large dogs over 30 pounds so the sooner you book a reservation the better. Please complete the boarding contract and bring it with you the day you board your pet. This provides us with all the information we need to ensure your pet has as much of a great vacation as you do!

Surgical Information Packet - Required​​​​​​​

Our greatest concern is you pet's wellbeing! Before putting your pet under anesthesia for surgery, we routinely perform a complete pre-surgical exam. We also highly recommend that a Pre-Anesthetic Blood Profile be performed on all patients undergoing anesthesia to maximize patient safety.

We realize surgery and anesthesia are scary for both the owner and patient and we attempt to make surgery day as safe and comfortable for all involved. The physical examination and blood work done prior to any surgical procedure allows us to best minimize anesthetic and surgical risks and maximize patient safety.

The Pre-Anesthetic Blood Profile helps alert our surgeon to the presence of anemia, infection, diabetes and/or kidney or liver disease that and could complicate the procedure. These conditions may not be detected without a pre-anesthetic blood profile thus not allowing for the most appropriate and safest anesthetic regime to be administered. These tests are similar to those your own physician would run if you were to undergo anesthesia. In addition, these tests may be useful if your pet's health changes to develop faster, more accurate diagnoses and treatments.

The Pre-Anesthetic Blood Profile includes:

  • CBC: PCV (anemia), White Blood Cell Count (infection), Red Bood Cell Count (anemia/bleeding disorder), Platelet Count (clotting disorder)
  • Profile: BUN and Creatinine (kidney), ALKP and ALT (liver), Glucose (sugar), and Total Protein (dehydration)
